| Dear Kidsaurus Parents, |

| Please read, sign and return the form below. |
| Agreement |
| I have completed the attached forms. In addition, I have read and understand the general |
| information. I understand tuitons payments are due by July 22th. I also understand that I |
| will be liable for the full tuition payment of Summer School Program once I return this |
| agreement. The tuition fee and the enrollment application fee are |
| non-refundable, and prorating or refunding tuition payment are not given for absences due to |
| illness, family vacations or schools holidays and breaks or any other reason. |

| Penalty Fee: |
| For parents who come late to pick the kid(s) up, ¥1000 for after 10 mins. |
| after 10ninutes late, ¥500 for every 5min. |
| Also, ¥1000 penalty per each day late if you did not cancel the whole |
| tuition by Jul. 17th. |
|
| Additional fee will be added for field trip expenses, ¥3000 material and ¥5,000 enrollment |
| for non-Kidssaurus students. |
| Kidsaurus is very careful during Summer School. However, we can not take any |
| responsability if an injury and/or accident happen to the child(ren) in and out of the |
| school. |
|
| Please bring every day , towel, hat/cap, UV sun lotion, your |
| bottle of drink and extra clothes. Kids are not allowed to bring any electronic games or |
| anything that has nothing to do with summer school. We will call the parents and ask |
| them to pick up the child(ren) if need(s) to go to the hospital or doctor. And also if we are |
| dealing with a behavior issue and need assístance to correct the bahavior. |

| 添付書類の必要事項に記入し提出致します。そしてきっザウルスのサマースクールの概要を |
| 読み、理解しました。参加料の支払い期限が７月22日であることも理解しております。 |
| また、この同意書の提出後、きっザウルスのサマー･スクールの参加料全額を支払うことも了承 |
| しております。 |

| お申し込み後の登録料と参加料は返金不可となっており、病気や家族旅行、学校の都合等による |
| 欠席に対する日割り計算や返金はされないことも理解してます。 |
|
| ペナルティー料金について：参加料の締切日(7月９日)以降のお支払いには、営業日1日につき |
| \1,000の遅延料が発生し、教室へのお迎えが10分遅れた場合は\1,000、以後５分毎に\500 |
| ずつ追加料金が加算されることを理解しています。 |
|
| 物品費用として\5,000、キッザウルス以外の生徒には入会金として3000円が必要であること、 |
| また校外学習の内容によっては、追加料金が発生する場合がございます。 |
| サマー・スクール中、きっザウルスでは安全に対し細心の注意を払いますが、教室内外でのケガ |
| や事故が起こった場合、きっザウルスでは責任を負いかねます。もしお子様が医者に診てもらう |
| 必要がある場合は、保護者の方にご連絡をし、お子様のお迎えをお願いすることがあります。 |
